Per MMQC-23-1239, the above link will route you to the new USAMMA DOC page to download the forms for requesting vaccine.  Please ensure the order forms are downloaded/saved to your computer for the features to work properly. 
 
USAMMA DOC will keep customers informed and updated once the Cloud migration has been completed.
